ABSTRACT

Abstract Objective Reporting our experience of the management and treatment of Idiopathic granulomatous mastitis (IGM) in a low-income country by describing patients characteristics and therapy with emphasis on conservative surgical excision and postoperative care as the cornerstone of treatment. Methods A retrospective cohort of women with histopathological diagnosis of IGM from 2014 to 2018 at Instituto Nacional Materno Perinatal in Lima, Peru. Patients' characteristics, clinical presentation, treatment, management, postoperative care, and follow-up were analyzed. Results Thirty-eight patients with histopathological diagnosis of IGM were identified. Their average age was 35.9 years and 23 (60.5%) reported previous use of hormonal contraceptives. Nine (23.7%) patients had chronic mastitis with previous treatment. The time from the onset of symptoms to the first clinic consult was 5.1 months on average. Twenty-one (55.3%) patients had the lesion in the right breast, with a mean size of 6.9 cm. Conservative surgical excision was performed in all patients. Additionally, 86.8% required corticosteroids and 78.9% were treated with antibiotics. Complete remission was obtained at 141 days on average (range 44 to 292 days). Six (15.8%) women reported ipsilateral recurrence and 5 (13.2%), contralateral. The latency time was 25.5 months on average. Conclusion The conservative surgical treatment demonstrated and close follow-up made for a high cure rate, but with recurrence similar to that reported in the literature. Use of gloves is an alternative to manage post operative wounds in a low-income country. The most frequent adverse effect was breast surgical scar.

ABSTRACT

RESUMEN OBJETIVO: Presentar la experiencia de la unidad de mama de nuestro hospital con la utilización de la ecografía intraoperatoria en el tratamiento de las lesiones no palpables de mama. PACIENTES Y MÉTODO: Se incluyeron aquellas pacientes con lesiones no palpables de mama y ecovisibles. Intraoperatoriamente se localizó la lesión con la ecografía y se procedió a su exéresis, con comprobación ecográfica de su correcta extirpación con márgenes de seguridad. Se realizó estudio macroscópico en fresco de los márgenes marcados con tinta intraoperatoriamente. En caso de que los márgenes no fueran correctos se procedía a una ampliación de márgenes en el mismo acto quirúrgico. RESULTADOS: Desde el año 2012 se han intervenido 52 pacientes. En todas las pacientes se localizó la lesión con la ecografía. Se realizó tumorectomía a 24 pacientes y a 28 pacientes se les asoció la biopsia del ganglio centinela. El resultado patológico definitivo fue de 19 lesiones benignas y 33 lesiones malignas. A una paciente se le realizó mastectomía simple por presentar un carcinoma in situ extenso con microinfiltración no diagnosticado con las pruebas radiológicas preoperatorias. El resto de pacientes presentaron márgenes libres de tumor. CONCLUSIONES: La ecografía intraoperatoria es una técnica simple y fácil de desarrollar. Presenta una baja tasa de afectación de márgenes y es enteramente cirujano-controlada. Es confortable para el paciente y conlleva un bajo riesgo de complicaciones relacionadas con la técnica.


ABSTRACT OBJETIVE: To present the results of our hospital's experience with the utilization of intraoperative ultrasound in the treatment of non-palpable breast lesions. PATIENTS AND METHOD: We included those patients whose breast lesions were non-palpable yet simultaneously visible on ultrasound. The lesions were located intraoperatively with ultrasound and were removed with ultrasound verification of the proper security margins. An examination of the intraoperative macroscopic margins with ink was done. In cases with incorrect margins, a re-excision was done utilizing the same technique but with amplified margins. RESULTS: Dating from 2012, we have operated on 52 patients. In all cases, the lesions were discovered and localized by means of ultrasound. Lumpectomy was performed on 24 patients and we associated the sentinel node biopsy in 28 cases. Subsequent pathology reports determined that 19 lesions were benign and 33 lesions were malignant. There was one patient with a mastectomy because a long extensive ductal carcinoma in situ with microinfiltration that was not seeing during the preoperative study. CONCLUSIONS: Intraoperative ultrasound is an easy and simple technique that is entirely surgeon controlled and results in a low rate of positive margins. The procedure is comfortable for the patient and carries with it a low rate of complications.

RESUMO Objetivo: avaliar o perfil de segurança e os resultados estéticos do 2-octilcianoacrilato versus sutura intradérmica com fio de nylon em cirurgias mamárias. Métodos: ensaio clínico randomizado, aberto, que avaliou a ocorrência de complicações, como deiscência, hematoma, infecção e reações alérgicas após o uso do 2-octilcianoacrilato ou do fio de nylon. Também foi analisado o tamanho das incisões, o tempo de fechamento da pele e o tempo cirúrgico total. O resultado estético foi avaliado após 40 e 180 dias da cirurgia, por meio da largura média da ferida operatória e por avaliação subjetiva conceitual (ótimo, bom, razoável ou ruim). Resultados: foram incluídas 79 pacientes, sendo 37 no grupo 2-octilcianoacrilato e 42 no grupo de sutura com fio de nylon. O estudo foi interrompido antes do término do recrutamento dos pacientes pela ocorrência de maior número de deiscências no grupo do adesivo (OR: 11,42; IC95%: 1,36-96,02; p=0,007). Em relação às demais complicações analisadas, ao tempo cirúrgico e ao resultado estético no pós-operatório, não se observaram diferenças significativas entre os grupos. A média do tamanho da ferida operatória foi maior no grupo do adesivo em relação ao grupo da sutura, mas não houve correlação entre o tamanho da ferida e o maior número de deiscências. Conclusão: o 2-octilcianoacrilato apresentou maior risco de deiscência em relação à sutura intradérmica, com resultados estéticos equivalentes.


ABSTRACT Objective: to evaluate the safety profile and aesthetic results of 2-octyl-cyanoacrylate versus intradermal nylon suture in breast surgeries. Methods: an open-label, randomized, clinical trial evaluating the occurrence of complications, such as dehiscence, hematoma, infection, and allergic reactions after the use of 2-octyl-cyanoacrylate or nylon thread. The size of the incisions, skin closure time, and total surgical time were also analyzed. The aesthetic outcome was evaluated at 40 and 180 days after surgery, by means of the average width of the surgical wound and by subjective conceptual assessment (optimal, good, reasonable, or poor). Results: 79 patients were included: 37 in the 2-octyl-cyanoacrylate group and 42 in the nylon suture group. The study was stopped before the end of patient recruitment due to the occurrence of a greater number of dehiscences in the adhesive group (OR: 11.42; 95%CI: 1.36-96.02; p=0.007). Regarding the other analyzed complications, the surgical duration and postoperative aesthetic result, no significant differences were observed between the groups. The mean operative wound size was greater in the adhesive group than in the suture group, but there was no correlation between wound size and the largest number of dehiscences. Conclusion: while the cosmetic outcomes with the two techniques were similar, there was a greater risk of dehiscence with the use of 2-octyl-cyanoacrylate compared to intradermal suturing.

Introdução: A mamoplastia de aumento é o procedimento cirúrgico estético mais realizado nos EUA e o segundo no Brasil. Simastia é uma de suas possíveis complicações. Esta é definida como o mal posicionamento medial dos implantes mamários, que cruzam a linha média, com perda do sulco intermamário. Métodos: Trabalho tipo série de casos. Duas pacientes foram submetidas à correção de simastia pósmamoplastia de aumento. A via de acesso utilizada foi a incisão inframamária prévia. A correção do sulco intermamário foi realizada por escarificação das superfícies anterior e posterior da cápsula, associada a pontos de adesão com fio monofilamentar não absorvível (4 linhas com 6 pontos cada, compreendendo uma faixa vertical de aproximadamente 4 cm de largura na região pré-esternal). Em um dos casos o implante foi recolocado imediatamente e, no outro, em procedimento realizado 3 meses após. Resultados: Obtivemos um novo sulco intermamário satisfatório, com adequado resultado estético em ambos os casos. Não se observou recorrência após 9 e 11 meses de observação. Conclusões: A abordagem com suturas de adesão para a redefinição do sulco intermamário foi efetiva para o tratamento da simastia pós-mamoplastia de aumento.


Introduction: Augmentation mammoplasty is the most commonly performed esthetic surgical procedure in the United States and second in Brazil. Symmastia is one of its possible complications. This is defined as incorrect medial positioning of the breast implants, which cross the midline, losing the intermammary sulcus. Methods: Work type, series of cases. Two patients underwent symmastia repair after breast augmentation. The access route used was the previous inframammary incision. The repair of the intermammary sulcus was performed by scarification of the anterior and posterior capsule surfaces, associated with adhesion suture with non-absorbable monofilament threat (4 lines with 6 points each, comprising a vertical band of approximately 4 cm wide in the presternal region). In one of the cases, the implant was immediately replaced, and in the other, the procedure was done 3 months later. Results: In both cases, we obtained a new satisfactory intermammary sulcus, with proper esthetic result. No recurrence was observed on follow-ups at 9 and 11 months. Conclusions: The approach with adhesion sutures to redefine the intermammary sulcus was effective in the treatment of symmastia after breast augmentation.

ABSTRACT

Descrever um caso de mama tuberosa com importante assimetria mamária, descrever a técnica e avaliar o resultado de caso. Diferentes técnicas foram usadas em cada mama, apesar de na literatura dizer que é importante empregar uma estratégia semelhante em ambas as mamas. A paciente evoluiu sem complicações e apresenta, com 6 meses de pós-operatório, resultado extremamente satisfatório, sem estigmas de mama tuberosa e com melhora importante da assimetria mamária. Na literatura é estabelecido que não há uma única técnica cirúrgica adequada para a correção dos diferentes tipos de malformações. A mama tuberosa representa um desafio nas cirurgias plásticas da mama e se tornam mais complexas quando a assimetria é mais exacerbada. Porém, o cirurgião que dominar as mais diversas técnicas mamárias estéticas e reconstrutoras poderá alcançar um resultado satisfatório.


To report a case of tuberous breast with significant breast asymmetry, describe the technique used and evaluate the outcome of the case. Different techniques were used on each breast, although studies in literature recommend the use of similar strategy in both breasts. The patient progressed without complications and had, 6 months after the surgery, extremely satisfactory result without tuberous breast stigmas, and significant improvement of breast asymmetry. The literature establishes that not only one surgical technique is adequate to correct different types of malformations. Tuberous breast constitutes a challenge in breast plastic surgery and it becomes more complex when the asymmetry is more severe. However, surgeons who is trained in a variety of aesthetic and breast reconstructive techniques can achieve a satisfactory result.

INTRODUÇÃO: Pacientes com gigantomastia apresentam múltiplos sintomas físicos e psicossociais. Várias técnicas foram propostas para o seu tratamento. A amputação mamária, descrita por Torek em 1922, apresenta com excelente alternativa, porém com prejuízo na funcionalidade mamilar e no formato da mama. Liacyr Ribeiro, em 1975, descreveu o retalho inferior dermoglandular não areolado a fim de proporcionar tecido de segurança para ressecção mamária, facilitar a montagem da mama e melhorar sua projeção. Este trabalho propôs unir estas duas consagradas técnicas, visando aprimorar os resultados das amputações mamárias. MÉTODOS: Foram operadas 11 pacientes com gigantomastia com prole definida, pela técnica de amputação mamária associada ao pedículo inferior não areolado. RESULTADOS: Distância média da fúrcula esternal ao mamilo foi de 35,6 centímetros na mama direita e 35 centímetros na mama esquerda, variando de 30 a 44 centímetros. A ascensão do complexo areolomamilar foi em média de 16,9 centímetros na mama direita e 16,4 centímetros na mama esquerda, variando de 12 a 25 centímetros. A quantidade de ressecção média de tecido mamário por paciente foi de 3559,5 gramas, variando de 1600 a 5890 gramas. A hipopigmentação do complexo areolamamilar esteve presente em três (27%) pacientes. A deiscência do T foi observada em dois (18%) pacientes. A não integração parcial do enxerto ocorreu em três (27%) das pacientes, com perdas estimadas de 10%, 30% e 80% do enxerto. CONCLUSÃO: A associação da amputação mamária com o pedículo dermoglandular inferior não areolado nos oferece um melhor remodelamento da mama, segurança na montagem desta, além de proporcionar uma adequada projeção da mesma.


INTRODUCTION: Patients with gigantomastia have multiple physical and psychosocial symptoms. Various techniques have been proposed for their treatment. Described by Torek in 1922, mammary amputation was presented as a great alternative, but resulted in reduced mammillary functionality and loss of breast format. In 1975, Liacyr Ribeiro described the use of dermaglandular inferior pedicle as safety tissue to allow for mammary resection, to facilitate breast assembly and to improve projection. The author proposed to unite these two consolidated techniques with the intention of improving breast amputation outcomes. METHODS: Eleven gigantomastia patients were operated on by means of the amputation technique using dermaglandular inferior pedicle. RESULTS: The mean distance between the sternal notch and the nipple was 35.6 cm for the right breast and 35 cm for the left breast, with all measures ranging between 30 cm and 44 cm. Rise of the nipple-areola complex was in average 16.9 cm for the right breast and 16.4 cm for the left breast, varying from 12 to 25 cm. The amount of breast tissue resection per patient was, in average, 3559.5 grams, ranging from 1600 grams to 5890 grams. Hypopigmentation of the nipple-areola complex was present in three patients (27%). Dehiscence of the T was observed in two patients (18%). Partial non-integration of the graft occurred in three patients (27%), with loses estimated at 10%, 30% and 80% of the graft. CONCLUSION: Associating mammary amputation with an inferior dermaglandular pedicle provides good remodeling and safe assembling of the breast, in addition to providing proper projection.

ABSTRACT

Abstract: Myiasis refers to Diptera larvae infesting vertebrate animals. There are two forms of the disease: primary and secondary. In primary myiasis, fly larvae invade and develop in healthy tissue; in secondary myiasis, flies lay their eggs in skin ulcerations, and the larvae develop in tissue necrosis products. Furuncular myiasis is a type of primary myiasis. Treatment for it consists of techniques such as the production of localized hypoxia to force the emergence of the larvae, and mechanical or surgical removal of the maggots. These techniques, however, are painful and often unsuccessful. We propose a new technique for extraction of myiasis larvae, which might facilitate the surgical procedure and constitute a virtually painless and aesthetic option for the patient.

ABSTRACT

A mamoplastia de aumento é um procedimento cirúrgico muito comum e seguro na cirurgia plástica, mas o mesmo não está isento de complicações. A galactocele após mamoplastia de aumento é raramente descrita na literatura. Este relato de caso refere-se a uma paciente de 34 anos de idade, que fazia uso de sulpirida há 2 anos e 4 meses e desenvolveu galactocele cerca de 100 dias após mamoplastia de aumento. O diagnóstico deve ser suspeitado quando se observar uma mama aumentada de volume, associada a calor local, dor ou desconforto mamário no pós-operatório. Acredita-se que a melhor conduta seja a drenagem cirúrgica, a fim de confirmar o diagnóstico de galactocele e excluir a presença de abscesso mamário.


Augmentation mammaplasty is a common and safe plastic surgery procedure, but it is not free from complications. Galactocele after augmentation mammaplasty is rarely described in the literature. We discuss the case of a 34-year-old woman who had been taking sulpiride for 2 years and 4 months and developed galactocele approximately 100 days after augmentation mammaplasty. However, diagnosis should be suspected if breast size increases and it is associated with postoperative local heat, pain or breast discomfort. We believe that the surgeon must surgically drain galactocele to confirm diagnosis, especially to exclude the presence of breast abscess.

ABSTRACT

Background: Rosai Dorfman disease is uncommon and consists in a benign fibrous and hematopoietic tissue proliferation, usually located in the head or neck. It is self-limited and the treatment is the surgical excision of the lesion. Case Report: We report a 69 years old woman consulting for a palpable right breast nodule with a mammography and mammary ultrasound informed as BIRADS 4 and 5, respectively. A core biopsy was informed as a mammary Rosai Dorfman disease. Two years later, due to a lesion growth, the patient was subjected to a partial mastectomy. The pathological study of the surgical piece confirmed the diagnosis of the core biopsy. This disease may resemble a malignant disease and the clue for its diagnosis is the pathological study.


Introducción: La enfermedad de Rosai Dorfman es una entidad infrecuente, que consiste en la proliferación benigna del tejido fibroso y hematopoyético ubicado generalmente en cabeza y cuello. Se trata de una enfermedad benigna, autolimitada, cuyo tratamiento generalmente se limita a la resección quirúrgica, con elevada tasa de recidiva. Objetivo: Exposición de un caso de Enfermedad de Rosai Dorfman mamario, inicial-mente sugerente de patología maligna. caso clínico: Mujer 69 años derivada de atención primaria por nódulo palpable en mama derecha, mamografía y ecografía mamaria BIRADS 4 y 5, respectivamente. Biopsia Core informa Enfermedad de Rosai Dorfman mamaria. A los dos años de diagnóstico se objetiva lesión de mayor tamaño. Se realizó mastectomía parcial con biopsia de pieza quirúrgica compatible con diagnóstico de Biopsia Core previa. Discusión: La similitud clínica de esta enfermedad con patologías malignas mamarias, asociado a su baja prevalencia, dificultan la pesquisa de casos como el expuesto. En los estudios de imágenes raramente se sospecha. La clave del diagnóstico son los hallazgos histológicos en el que característicamente se observa emperipolesis con tinciones inmunohistoquímicas positivas para S 100 y negativo para CD 1. Dado que es una enfermedad benigna y autolimitada su tratamiento generalmente se limita a la resección quirúrgica. Este permite extirpar el tumor en estudio, realizar el diagnóstico, y aliviar la sintomatología producida por el efecto masa de la lesión. A pesar de lo anterior la recidiva es alta, por lo que el seguimiento debe ser a largo plazo.

ABSTRACT

Association between postoperative nausea and vomiting (PONV) and micro-opioid receptor A118G single nucleotide polymorphism (SNP) is undefined and might underlie inconsistent results of studies on PONV occurrence in patients undergoing general anesthesia with the opioid, remifentanil. Four hundred and sixteen Korean women undergoing breast surgery with general anesthesia were randomized to receive remifentanil 10 ng/mL (plasma-site, Minto model) using a target-controlled infusion device and either propofol for total intravenous anesthesia (T group) or sevoflurane for inhalation anesthesia (I group) with bispectral index values maintained between 40 and 60. Blood specimens were collected after anesthesia induction for A118G SNP analysis. PONV and postoperative pain were evaluated. A118G SNP type distribution among Korean female adults studied was AG (n=195)>AA (n=158)>GG (n=63). Regardless of anesthetic technique, patients with GG types had lower PONV scale on arrival at postoperative care unit (PACU) (P=0.002), while T group showed lower PONV scale than I group up to 6 hr after PACU discharge in AA and AG types. No differences were apparent for postoperative pain among opioid receptor polymorphism. PONV occurrence differs according to opioid receptor polymorphism and anesthetic technique in patients undergoing general anesthesia with remifentanil.

La hiperplasia pseudoangiomatosa (PASH) es una lesión proliferativa benigna de la mama, poco frecuente, caracterizada por la existencia de lagos pseudovasculares embebidos en una gran proliferación del estroma mamario. Probablemente, el desarrollo de la PASH tenga una influencia hormonal, por lo que típicamente se diagnostica en mujeres en edad fértil. La PASH es un hallazgo histopatológico casual en las piezas quirúrgicas y biopsias mamarias realizadas por otra patología. La presentación clínica en forma de masa palpable es poco frecuente. El principal diagnóstico diferencial debe realizarse con el angiosarcoma de bajo grado. El tratamiento de la PASH nodular es una correcta exéresis quirúrgica asegurando borde sano amplio. El pronóstico es excelente, con un mínimo riesgo de recidiva si se realiza una adecuada cirugía. Se presenta el caso de una mujer de 37 años que acude a consulta por un nódulo mamario de crecimiento rápido.


Pseudoangiomatous stromal hyperplasia (PASH) of the breast is a rare benign proliferative mesenchymal lesion characterized by the presence of open slit like spaces embedded in a hyalinized fibrous stroma. The development of PASH is probably subject to hormonal influence so it typically affects women in the reproductive age group. Pseudoangiomatous stromal hyperplasia is frequently an incidental histologic finding in breast surgeries or biopsies performed for other injuries. In rare cases, it presents as a localized breast mass. The most important differential diagnosis is low-grade angiosarcoma. Tumorous PASH is treated by local surgical excision with clear margins. The prognosis is excellent, with minimal risk of recurrence after adequate surgery. The presented case was a 37-years-old woman who was admitted with a rapidly growing breast tumor.

Introducción: Los hamartomas y la hiperplasia estromal pseudoangiomatosa nodular (PASH) son entidades infrecuentes en la patología tumoral mamaria, sin embargo, pueden simular al cáncer de mama. La única herramienta certera preoperatoria es la biopsia. El tratamiento quirúrgico es curativo con bajo índice de recurrencia. Objetivo: El objetivo de este trabajo es exponer un caso de hamartoma mamario con PASH asociado cuya presentación inicial fue sugerente de probable patología maligna. Caso Clínico: Mujer de 44 años, ingresa a controles por probable patología mamaria maligna, la mamografía fue informada como BIRADS 0, la ecografía mamaria como BIRADS US: 5, la biopsia CORE informó PASH. Se realizó mastectomía parcial objetivando en la biopsia diferida un hamartoma mamario con extensa hiperplasia estromal pseudoangiomatosa. Discusión: Los hamartomas y la PASH son patologías sin presentación clínica específica, los hallazgos radiológicos del hamartoma pueden ser patognomónicos, no así en la PASH; en el caso expuesto el estudio anatomopatológico demostró una asociación de estas patologías, la cual se describe desde un 16 por ciento a un 71 por ciento de los casos de hamartoma mamario. El análisis de toda la pieza quirúrgica es fundamental para establecer un diagnóstico definitivo.


Introduction: Hamartoma and nodular pseudoangiomatous stromal hyperplasia (PASH) are inusual breast tumor entities, however, can simulate breast cancer. The only accurate tool is preoperative biopsy. Surgical treatment is curative with low recurrence rate. Objective: To present a case of mammary hamartoma associated with PASH whose initial presentation was suggestive of probable malignancy. Case Report: Woman, 44 years old, admitted to controls because of probable malignant breast disease, mammography was reported as BIRADS 0, breast ultrasound as BIRADS U.S: 5, CORE biopsy reported PASH. Partial mastectomy was performed. On delayed biopsy mammary hamartoma with extensive stromal hyperplasia pseudoangiomatosa was diagnosed. Discussion: PASH and hamartomas are diseases with no specific clinical presentation, radiological findings may be pathognomonic of hamartoma, while not in PASH. In the reported case the pathological study showed an association of these conditions, described in a 16% to 71% breast hamartoma cases. The analysis of the entire surgical specimen is essential for a definitive diagnosis.

A survey study was done during the period between September 2010 and December 2012 on 2580 Friesian cows and heifers distributed in four armed forces farms.This work revealed that, teat wounds represent the higher acquired teat affections [28.16%] while teat papilloma represents the lowest affection [1.65%]. The supernumerary teat represents the higher congenital teat abnormalities [36.8%] while the lower was incompetent teat [1.5%].The present study revealed that the prevalence of acquired teat affections were in November, December, January, and February, whenever the weather is cold and rainy and stables are muddy and the accidental injuries during milking, feeding or even laying were predominant and predictable

Objetivo: Apresentar experiência com o uso de aparelho dedicado para radiografia de espécimes de biópsias cirúrgicas de mama (Faxitron®), com ênfase na confirmação do achado do alvo cirúrgico na amostra tecidual. Métodos: De março de 2010 a março de 2011 foram realizadas 112 biópsias radioguiadas de lesões nao-palpáveis de mama, nas quais houve necessidade de radiografia da peça operatória para avaliação do acerto da retirada do alvo cirúrgico (microcalcificações, ,nódulos ou clips aplicados após mamotomia). Todas as lesões foram marcadas pelo método radioguiado (ROLL) e as radiografias da peça feitas por meio de aparelho dedicado, localizado no centro cirúrgico. Resultados: As imagens foram obtidas em tela de computador quase que instantaneamente, após cerca de 30 segundos, e tiveram excelente qualidade visual. O alvo cirúrgico esteve presente na primeira amostra em 109 dos 112 casos (97,3%), sendo que nos demais foi necessária pelo menos mais uma ampliação da biópsia para a remoção da lesão. Conclusões: A técnica de biópsia radioguiada de lesões nao-palpáveis de mama é altamente eficiente, mas a monitorização radiográfica da peça é fundamental para assegurar o acerro na remoção do alvo cirúrgico. A radiografia do espécime cirúrgico de biópsias de mama pelo aparelho Faxitron® é fácil, segura e muito rápida.


Objectives: To report the experience with the use of a dedicated machine for breast specimens radiography (Faxitron®). Methods: From March 2010 to March 2011, 112 radioguided breast biopsies (ROLL) for non-palpable lesions were performed. In all cases, the excised tissue samples were radiographed by the dedicated equipment, inside the Surgical Center area. Results: The images were seen in a computer monitor screen almost instantaneously (after about 30 seconds) and showed excellent visual quality. The surgical targets were observed in the first specimen in 109 of the 112 cases (97.3%). In the other cases, supplementary excisions were needed to allow the target removal. Conclusions: The radioguided occult lesion localization technique for non-palpable breast lesions is highly efficient, but the specimen radiographic confirmation is mandatory to check the target removal. The radiography of breast biopsies specimens through the equipment (Faxitron®) is easy, safe and very fast.

ABSTRACT

Gigantomastia é uma anomalia que causa complicações sérias e nem todas as macromastias são sintomáticas. Dessa forma, avaliação apropriada da paciente, bem como atendimento multidisciplinar, faz-se necessária. A intervenção cirúrgica é conseqüentemente o método de escolha nessa patologia, sendo uma excelente terapêutica na gigantomastia. O tamoxifeno pode ser um adjuvante útil e permitir resultados estáveis quando combinado com a mamoplastia redutora.


Gigantomastia is a pathology that can cause serious complications but some cases there are no symptoms. An adequate evaluation of the patient and multi-disciplinary attendance is necessary. The surgery intervention is method of choice in this pathology, being an excellent therapeutic. Tamoxifen can be a good adjuvant producing stable results when combined with the cosmetic surgery.

ABSTRACT

O tratamento cirúrgico de neoplasias mamárias benignas e malignas, assim como a abordagem de hipertrofias e assimetrias mamárias, requer do mastologista conhecimentos técnicos anteriormente utilizados apenas na área da cirurgia plástica. As técnicas de mamoplastia redutora utilizadas nos dias atuais derivam de técnicas descritas há muitos anos, com diferentes adaptações, objetivando sempre melhores resultados imediatos e tardios. As alterações estruturais e cicatriciais provocadas por esse tipo de cirurgia devem ser bem conhecidas e acompanhadas quando for empregada na solução de problemas oncológicos. Nesta breve revisão, discutem-se diversos aspectos técnicos e conceituais referentes à mamoplastia redutora com relevância na clínica mastológica atual.


New trends in breast cancer surgery motivated breast surgeons to get acquainted with surgical skills formerly only employed in aesthetic surgery. The modern surgical approach to reduction mammaplasty represents a surgical procedure improvement for better shape and reduced scars. Postsurgical changes, some of which may mimic cancer must be recognized and followed by mammography and clinical examination. In this review, we go through some issues about reduction mammaplasty and its importance for breast surgeons.

ABSTRACT

A incidência de lesões não-palpáveis da mama vem aumentando através dos tempos, fato intimamente correlacionado à difusão da mamografia de alta resolução. Para tanto, tornam-se necessárias marcações pré-operatórias para localização dessas lesões, por meio de técnicas com fio metálico, material radioativo, carvão ativado ou, como no nosso serviço, a utilização de corante vital (azul patente). Apresentaremos a experiência do Hospital e Maternidade São Cristóvão na utilização do corante azul patente para a marcação de lesões não-palpáveis da mama, abordando em concomitância a localização do linfonodo sentinela em alguns casos. Selecionaram-se as pacientes que haviam sido submetidas à marcação pré-operatória com azul patente no Hospital e Maternidade São Cristóvão, no período de julho de 1999 a março de 2007, totalizando 285 pacientes com lesão (ões) não-palpável (eis) da mama. Foram 332 marcações, por orientação mamográfica, injetando–se 1 mL de azul patente intratumoral, 30 minutos a três horas antes do procedimento cirúrgico. A lesão foi excisada e confirmada por radiografia da peça operatória e/ou congelação intra-operatória. Houve 160 casos que tinham indicação para realização do linfonodo sentinela (LS), sendo realizado, nessas pacientes, um reforço da dose de azul patente na sala operatória, acrescentando-se 1 mL do corante na região subareolar e biópsia do LS com congelação intra-operatória deste. Procedeu-se a dissecção axilar completa somente nos casos em que o LS foi positivo. A idade média das pacientes foi de 60,92 anos (23 a 89 anos). As lesões foram identificadas em todos os casos (100%) e totalmente excisadas sem intercorrências cirúrgicas. Nos casos indicados de linfonodo sentinela, a taxa de localização concomitante do LS foi de 98,13%. O tamanho médio dos tumores foi de 1,3 cm. A avaliação histopatológica em parafina revelou 133 casos malignos (40,1%), 26 lesões precursoras de risco (7,8%) e 173 tumores benignos em geral (52,1%). A localização...


The incidence of non-palpable breast lesions is increasing due to the diffusion of the high resolution mammography. For surgical biopsies orientation there are several techniques, such as the insertion of a metallic hook wire and the injection of radioisotopes or corants. The aim of this paper is to present the of the Hospita and Maternity São Cristóvão experience with blue corant for marking occult breast injuries allowing concomitantly the localization of sentinel node (SN). There were 332 markings under, mammography and/or ultrasonograph orientation, with the injection of 1 ml of bluedye intra-tumoral, from 30 minutes to 3 hours before surgical procedure. The lesion was excised and confirmed by specren radiography.In 160 cases there was indication of SN biopsy. In these patients it was injected, a reinforcement of the dose of the blue patent just in the surgical room, adding more l ml of the blue dye in the subareolar region. There average age of the patients it was 60.9 years (23 – 89). The non-palpable breast lesions were identified in all of the cases (100%) without any surgical complication. In the cases of SN biopsy, the rate of localization of the SN was 98,1%. The average size of tumors was 1.3 cm ou the histopatological evaluation in paraffin sections being the results as follows: 133 malignant (40.1%), 26 pre-malignant injuries of risk (7.8%) and 173 benign lesions (52.1%). The localization of occult lesions using blue patent - COLL (Colour Occult Lesion Localization) - and concomitant sentinel node in breast tumors are practical, safe and confident allowing, treinament with minimum aggression and satisfactory aesthetic result. As the radiocolloids method, this is a methodology of high sensitivity, with the advantage of the low cost, being able to be used in hospitals without nuclear medicine services.

ABSTRACT

Las microcalcificaciones mamarias son las lesiones no palpables de la mama más frecuentes. Serevisaron de forma retrospectiva todas las fichas clínicas de las pacientes con lesiones mamarias nopalpables visualizadas por mamografía, que concurrieron de febrero a diciembre del año 2001 alConsultorio de Mastología del Hospital Central del Instituto de Previsión Social (HCIPS). Seestudiaron las pacientes que presentaban microcalcificaciones quienes fueron clasificadas y tratadassiguiendo las pautas del Colegio Americano de Radiología. Las pacientes de alta sospecha y las deintermedia con antecedentes familiares de cáncer de mama, fueron sometidas a biopsia radioquirúrgica.En las de baja e intermedia probabilidad de malignidad sin antecedentes familiares, seadoptó una conducta conservadora. De las 106 pacientes con lesiones no palpables de la mama, 78presentaron microcalcificaciones, de ellas 36 (46,2%) fueron de baja, 9 (11,6%) de intermedia, y33 (42,2%) de alta sospecha de malignidad. Al inicio del estudio, 36 pacientes y a los seis meses12 más (seis de baja probabilidad y seis de intermedia sin antecedentes familiares) fueronsometidas a biopsia radio-quirúrgica. Los hallazgos anátomo-patológicos demostraron carcinomamamario en 24 casos (50%), cuatro de ellos invasores (tres con axila comprometida) y 20 noinvasores. Las microcalcificaciones mamarias fueron el hallazgo radiológico más común entre laslesiones no palpables de la mama. Su reconocimiento y caracterización es elemental para detectarprecozmente patologías malignas de la mama
